Troubleshoot Apache or database load issue

Our web server (Apache + PHP-FPM + MySQL on an 8-core, 16 GB instance) is crashing under high traffic load, and we haven’t been able to isolate the bottleneck — it looks like something between FPM process limits and MySQL contention.
