Hello, We have one survey with about 212 question that are in 24 groups. The survey is build in LimeSurvey Community Edition Version 6.8.1+241120. Not all the question are mandatory, some of them have Conditions based on previews question, same for the groups. We also have some question where people can select one or multiple images. What we need? We need someone to help us setup a plugin to export the results in PDF. The most important task: The pdf should only contain the questions that someone answer, and also the images that people select. We don't want to have the unanswered questions! The second important task: The PDF need to be branded with our custom header we can provide it in word/jpg/png etc. and in the end of the PDF we want to add a value from a field and signature field. (Name that they complete and signature zone) The third important task: add page number and total pages of the pdf. Fonts should be simple, and we don't want any background because most of the time will be printed on paper so we want to save ink and paper. The forth most important thing: The script/plugin should work on any forms, not specific to this one. Because we usually update the form and add or delete questions. What we would do for you: We will setup a beta environment if needed and load the survey after that we will deploy on our day to day page. We are open to any advice, if you think is necessary. Please read carefully. If you don’t have experience with limesurvey please don’t waste both of us time. We are open to change the platform to a new platform that you know only if they can do all of this and be self hosted, and no recurring payment for platform.
