If you think adding S-M-L inside those categories helps clarity, feel free, but the main requirement is that all common youth-to-adult sizes are represented. Once a coach submits the form, the data should arrive in a tidy spreadsheet (download link or auto-emailed CSV/Google Sheet is fine) so I can forward it straight to our kit supplier. A confirmation message to the coach is useful, but again—no checkout, no money collection. Deliverables: • Responsive online form styled in club colors (I’ll supply the hex codes). • Dropdown controls for every field, including the size selector covering Youth, Adult, XL. • Serverless or light-backend solution that compiles submissions into a single spreadsheet (CSV, Google Sheet, or similar). • Simple admin link or password so I can export results anytime.
