The form must collect: • Name & email (single field pair) • Service required (dropdown I can edit easily) >the state they need the service for and the state they live in (drop down selection) • Detailed message Note: each service listed that they can choose from will get its own auto response giving detailed information on the specific service they requested. With this auto response, they should be able to reply to the email for connection to a live person. Please build it with straightforward, clean styling—white background, subtle borders, nothing flashy—and make sure it works responsively on phone and desktop. Basic client-side validation, a simple anti-spam measure (honeypot or reCAPTCHA v2-“I’m not a robot”), and clear success/error messages will complete the job. Deliver the fully commented code plus a short read-me so I can drop the files into my existing site and update the service list later if needed. The forms platform used must be stable and easily editable.
