I’ve drafted all of the questions for a simple contact-capture form and now need help turning that draft into a polished Google Form that drops every submission straight into our Housecall Pro account. Form specifics • Purpose: collect basic contact details—name, email and phone—while guiding the visitor through a short set of multiple-choice questions. • Logic: certain follow-ups should appear only when a particular option is chosen, so conditional logic must be enabled. • Presentation: the finished form has to blend into our website via the Google Forms embed code. Zapier connection Once the form is live, I’ll need a Zap (or multi-step Zaps, if required) that captures each new response and creates or updates the corresponding customer record inside Housecall Pro. All fields must map cleanly, and no duplicate customers should be generated. Deliverables 1. Live Google Form, styled to match the site and tested for conditional flows. 2. Embed snippet plus instructions on where to paste it. 3. Working Zapier workflow(s) with clear field mapping and a quick screencast or screenshots showing a successful test submission flowing into Housecall Pro. The job is complete when I can submit a test entry on the embedded form and watch it appear exactly where it belongs in Housecall Pro with no manual touch-ups.
