I already have the basic questions in place, but I need it polished and extended so it behaves like a true feedback-and-scoring application, not just a survey. Scope of work • Feedback submission must feel seamless, with conditional sections and clear progress indicators so respondents never get lost. • A custom scoring system has to run behind the scenes. Each response should be weighted, totaled, and stored automatically—ideally through Apps Script tied to the form’s response sheet. • Once scores land in Sheets, I want instant, lightweight data analysis: automatic dashboards, color-coded status, maybe charts that refresh on form submit. Design + functionality Both aesthetics and logic matter here. I’m open to your suggested CSS tweaks (via Forms add-ons or embedded HTML) as long as the finished form stays clean, on-brand, and responsive. At the same time, the underlying script must remain maintainable, clearly commented, and free of quota-busting loops. Feedback form to reset after submission. Template for each assessor.
