I’m after a straightforward survey form I can drop straight into my site. The only input I need from visitors right now is their Name, and each submission must trigger an email notification to me with the captured data. Please set it up so I can easily paste the final HTML/CSS (and any light JavaScript or PHP you use) into an existing page, or alternatively provide a link to a lightweight hosted version I can iframe. A clean, responsive layout is important, and I’d like clear commenting in the code so I can tweak styling later if needed. Deliverable: working form code (or hosted link) plus a brief read-me showing where to set the destination email address. That’s all—quick, simple, and ready to launch.
