The form must gather personal information only—specifically the customer’s full name, email address, telephone number, collection postcode and required collection date. All entries should be stored securely in the site’s database so I can view, filter and export them later for reporting or compliance purposes. Once a collection is marked complete, the system must generate a data-destruction certificate (PDF is fine) that merges the customer’s details and a unique reference number, then emails it to the customer automatically. No other delivery methods are needed.
